 ADVANCE MEDICAL DIRECTIVES

As a free service to our community we offer assistance in helping you prepare your Advance Medical Directives such as Living Wills, and the Health Care Power of Attorney.  If you are age 18 or older and mentally competent you have the right to make decisions about your medical care.  Advance Directives are legal forms that record those decisions.  If need assistance in completing your Advance Directive or if you have questions please feel free to contact us at 843-374-6127.
