Admitting/Registration Services OPEN ADMISSION POLICY It is the policy of Lake City Community Hospital to admit and treat all persons without regard to sex, handicap, national origin or religious creed. Admission requirements are the same for all persons. ADMISSIONS/REGISTRATION- INPATIENT AND OUTPATIENT DIAGNOSTIC The Admitting/Registration Service Area is located on the first floor of the hospital. You should check in at the Admission desk and you will be directed to a registration booth where a representative will collect information, such as your name, address, and insurance/payment information. This information is required by law and by private insurance companies as well as Medicare and Medicaid. We appreciate your patience and cooperation during this process. Federal regulations require hospital personnel to inquire as to the presence of an Advance Directive for all inpatient and surgery patients. This includes a Living Will or a Healthcare Power of Attorney. Information on Advance Directives is provided to patients upon admission. You will be asked to sign various forms and then an identification bracelet will be prepared and placed on your wrist. The Admission/Registration Staff will then direct you to the appropriate department. PLEASE BRING THE FOLLOWING: · Physician’s Order · All active insurance cards · Picture ID / Social Security Card · Advance Directive (Living Will or Healthcare Power of Attorney) if you have one. TIPS: Patients are encouraged to pre-register if possible which allows the hospital time to obtain all necessary registration information, thus reducing the time spent registering. This also allows time to verify insurance benefits and obtain pre-certification approval from your insurance company. Please arrive for services at least 20 minutes prior to your appointment time. This will assure that you arrive at your appointment at the scheduled time. Please leave your valuables at home for safekeeping. EMERGENCY DEPARTMENT REGISTRATION: Upon arriving for treatment in the Emergency Department you will present to the registration desk where a clerk will log you in and call the triage nurse to assess you. After the triage nurse has assessed you the nurse will start the registration process by retrieving basic information. After you have received treatment in the Emergency Department you will be directed to our registration desk where your registration will be completed. We make every effort to treat our patients as quickly as possible. There are times when the emergency department is extremely busy with critically and seriously ill patients, and you may be required to wait for a period of time. Should you feel that your condition has changed, please check with the triage nurse and ask that they reassess you.
